Chronic Disease Management (CDM)

Free for all eligible patients.

The CDM programme is a free programme between the HSE and General Practice that aims to provide high quality, structured care for patients with specific long-term illnesses. This programme aims to keep people well and reduce hospital presentations and admissions. These illnesses include:


  • Asthma

  • Chronic Obstructive Pulmonary Disease (COPD)

  • Ischaemic Heart Disease

  • Atrial Fibrillation

  • Heart Failure

  • Type 2 Diabetes

  • Stroke/Transient Ischaemic Attack (TIA)


Patients aged 18 and over with a valid GMS/DVC and one or more of the above chronic diseases are eligible for twice yearly review with the GP and any blood tests, blood pressure monitors or ECGs required are performed free of charge.

Contraception
/ Women's Health

A Free Contraception Service is available to all patients aged 17 to 35 years (up to their 36th birthday) who have a valid PPS number. The scheme covers consultations to discuss contraceptive options, prescriptions for eligible contraceptives, and the fitting and removal of long-acting reversible contraceptive (LARC) devices.


At Kinvara Family Practice, we provide advice on all contraceptive options and can prescribe the oral contraceptive pill, contraceptive patch, NuvaRing® and Depo-Provera® injections. We also fit and remove Implanon NXT® implants ("the bar"), Mirena®, Kyleena® and Jaydess® intrauterine systems (IUS), as well as the Yanae copper intrauterine device (IUD). The Yanae copper coil can also be used as a highly effective form of emergency contraception.


Contraceptive medications and devices covered under the scheme can be collected free of charge from your local participating pharmacy.


Patients aged 36 years and over who do not hold a valid Medical Card or GP Visit Card will be charged the usual consultation and prescription fees.

Antenatal / Postnatal Care (6-7 visits and 2 postnatal visits funded by HSE)

Kinvara Family Practice are signed up the Mother and Infant Care Scheme and are delighted to provide antenatal and postnatal care to all women. This scheme offers up to 6 antenatal visits and 2 postnatal visits to pregnant women. We also provide recommended antenatal vaccines including the annual flu vaccine and the pertussis (whooping cough vaccine) free of charge.

Appointments that are not related to your pregnancy will be charged in the usual way according to the practice fees.

Menopause Care

Patients are advised to book for a blood test (€30) followed by an extended consultation 2 weeks later (€90).

We are delighted to provide assessment and treatment of menopausal symptoms. Patients are recommended to book in advance for a blood test, followed by an extended GP consultation 2 weeks later. Blood tests look for other causes of menopausal symptoms including anaemia or thyroid disease. A hormone test (FSH) may also be useful for women aged <45 years of age.


At your consultation, you will be given a menopause questionnaire and a symptom checklist to complete in the waiting room. During your extended consultation, the GP will discuss your symptoms, your suitability for different treatments and your general, cardiovascular and bone health. Potential treatments include both hormone replacement therapy (HRT) and non-hormonal medical treatments. Most medications for menopause are now provided free of charge for all patients from your local participating pharmacy.

Cervical Screening


We are pleased to offer a free cervical screening service (previously known as a smear test) to all women aged between 25 and 65 years. This test checks the health of your cervix, the neck of your womb and looks to see if you might be at greater risk of developing cervical cancer.


Cervical screening helps to prevent cervical cancer by looking for the presence of human papillomavirus (HPV) - some types of HPV can lead to cell changes in your cervix and cancer – and abnormal cell changes in your cervix which if untreated, could turn into cancer.
