New Patients
To provide for your care we need to collect and keep information about you and your health in your personal medical record. Please complete the following form carefully. The information will be used to create your personal medical record on the practice computer. If you would prefer to submit the form in writing, please contact reception for a form.
Our practices are consistent with the Medical Council guidelines and the privacy principles of the Data Protection Acts. For further details please see our Privacy Statement.
Currently, we are not accepting new patients for the month of July 2026. We invite you to express your interest in joining the practice by completing this form. We will review our waiting list at the beginning of August 2026 at which point we may invite potential patients to attend a ‘meet and greet’, consultation with a General Practitioner.
The practice reviews applications based on clinical capacity, catchment area, continuity of care considerations, existing family registrations and the healthcare needs of the applicant. Completion of a registration form does not guarantee acceptance onto the practice list.
Please note that adults must complete their own separate form to register, however children can be included on a parent's application.
